Case Management

A Community Health Worker is a frontline public health worker, working alongside childbearing women and women with children up to the age of 18 months. Assists clients in their homes, community, or clinic setting. A CHW build trusting relationships with clients and connect those clients to care while eliminating barriers. A CHW is a trusted member of the community who advocates for individual and community needs, educating on maternal and child health information.
