Chief Executive Officer

General Summary

The CEO is responsible for providing strategic leadership for the organization by working with the Board of Directors and the executive management team to establish and execute long-range goals, strategies, plans, and policies, and to ensure the fiscal soundness of the organization.

Essential Job Functions

General Responsibilities

• Oversees the development and execution of short and long-range plans and goals for all facility programs, funding, management, and operations.
• Abide by a strict code of confidentiality and integrity in all matters related to the position.
• Represent the agency at community meetings, workshops and training seminars as required.
• As an ex-officio member of the Board of Directors, facilitates the scheduling of Board meetings and works with the Board Executive Committee to prepare and distribute the agenda and required materials in advance of all regular Board meetings. Ensures there is ample meeting space for Board meetings, including technology for remote meeting participation as needed, and supplies Directors with all information for the execution of their duties.

Peer/Prevention Specialist

Southwest Community Connections

Under the supervision of the Manager of Community Services, the Prevention Specialist works with a group of students 5-17 years of age in after school, school breaks and summer programs. The program is based on the 8 essential features of positive youth development programs (Physical and Psychological Safety, Appropriate Structure, Supportive Relationships, Opportunities to Belong, Positive Social Norms, Support for Efficacy and Mattering, Opportunities for Skill Building and Integration of Family, School and Community Efforts). The Prevention Specialist implements and supervises activities in 4 core areas: Academic Enrichment, Career Development, Cultural Enrichment and Personal Development & Wellness. Wellness encompasses alcohol, tobacco, and drug abuse prevention amongst other traditional wellness focus. Hours of operation Monday- Friday 10am-6pm, occasional Saturdays, school breaks 10am-6pm and summer camp July and August 9am-530p.

Accountant

General Summary

The Accountant will assist the CFO by recording, analyzing, interpreting, and reporting financial statistical information. Assists in all transactions affecting payroll, AR, AP, and general ledger. May also assist in budgeting.

Essential Job Functions

• Maintains records of financial transactions by establishing accounts; posting transactions; ensure legal requirements compliance.
• Maintains subsidiary accounts by verifying, allocating, and posting transactions.
• Reconcile and balance all accounts.
• Maintains historical records by filing documents.

Mainstream - Direct Support Professional

Southwest Community Center

General Summary
20+ hours a week with some evenings and weekends required.

  • All applicants must have the ability to respect people with developmental disabilities, including those from diverse cultural, ethnic and socioeconomic backgrounds.
  • Must be available Monday through Friday, on days of early school dismissal, during occasional day/evening trips and on weekends.

Essential Job Functions

  • Supervise OPWDD participants during all activities and events using an asset based approach
  • Assist in planning and implementing events, trips and activities for the program participants.
  • Maintain accurate and confidential records in a timely and manner.
  • Work Individually as well as on a team.
  • Communicate effectively with parents, participants, providers and staff.
  • Assist participants with toileting and hygiene as needed.
  • Abide by a strict code of confidentiality in all matters related to the position
  • Attend meetings, workshops and training seminars as required
  • Perform all other related and additional duties as required

Education Requirements
Minimum requirements for the position are a High School Diploma and 1-3 years related experience, Or any equivalent combination of education, experience, and training.

Previous Experience Requirement

  • Experience working with individuals with developmental disabilities. Experience working with youth.
  • Experience planning and assisting with recreational activities preferred. Experience in minimal to maximum assistance with personal hygiene preferred.

Knowledge and Skills Needed to Perform Effectively in this Position

  • Above average interpersonal and communication skills.
  • Ability to effectively multi-task with a high degree of accuracy.
  • Must possess and emit positive energy and the ability to remain motivated.

Additional Requirements

Linkage and Navigation Specialist

Facing Adversity (in our) Community (thru) Education (and) Support (FACES) is a Syracuse Community Connection’s program designed to provide HIV/AIDS awareness and quality High Impact prevention services to communities of color in Onondaga County. Under the direction of FACES Program Director, the Linkage and Navigation Specialist will ensure HIV positive clients and high-risk negative clients are assessed, referred, and linked to HIV/STD/HCV testing, mental health, and substance use counseling, link to Pre-Exposure Prophylaxis (PrEP), and other support services they may need. The Linkage and Navigation Specialist will provide client centered linkage and navigation and retention services in accordance with New York State Department of Health guidance to ensure clients’ needs are met in a compassionate-culturally competent manner. Linkage Specialist will be able to be comfortable in administering HIV testing. The Linkage Specialist will ensure all data pertaining to program related activities s/he is responsible to collect meets agency protocol and that data is timely entered into the New York State Department of Health-AIDS Institute Reporting System (AIRS). The Linkage Specialist is also expected to support and assist in all FACES and agency sponsored programs and events. This also includes participating in different outreach canvassing opportunities in local communities. Under the guidance of the Program Director the Linkage Specialist work to provide linkage and navigation services to communities of color affected and impacted by HIV. The Linkage Specialist also ensures program goals are met, client files are kept organized and updated, that service plan, and outcomes are thoroughly documented. The Linkage Specialist actively participate in the planning and execution of outreach program activities designed to meet work plan goals, which include facilitating support groups; (i.e.-CLEAR & Healthy Relationship) evidence-based interventions to HIV positive and high-risk negative clients.
